Holbeton Primary School – After School Club

Please complete this booking and consent form to book your place/s on the Summer term of the after school club for Holbeton County Primary School.

The children will be collected from the school gate by Hannah and Tara for two hours of fun forest school activities, including crafts, games, campfire cooking, den building, fire lighting & stream play, as well as hot chocolate and a biscuit snack.  All children will be dropped off at the school gates at 5.30pm.

 

Dates: Mondays from 15th May for 5 sessions (15th, 22nd May, 12th & 26th June, 10th July)

Time: 3.30pm – 5.30pm

Cost: £42.50 for the first child and £77.50 for two children for the 5 week term, (which works out at just £8.50 per child and £7 for the second sibling, per session).

By completing the booking you are consenting to:

• I give consent for my child to take part in the activities.
• I give my consent for A Touch of the Wild to use photos and/or video footage taken during the club to be used for publicity and marketing materials.
• I give my consent for A Touch of the Wild staff to administer first aid treatment and over the counter medication to my child/children if required (including plasters and anti-histamine).
• In the unlikely event of an accident, I consent for my child/children to receive emergency medical treatment
• If my child takes part in Forest School during school hours, I give A Touch of the Wild permission to care for them at the woodland Forest School site, between 3pm & the after school club start time of 3.30pm.
